Benefit Changes Allowed During Annual Enrollment Period All benefit elections made during this period are effective January 1, 2013.
2013 Health Insurance Premiums Premiums listed do not apply to those who are required to pay the less than half-time rates or the total premium.

Uniform Benefit Changes • Health plans will offer incentives for you to take a Health Risk Assessment (HRA) and Biometric Screenings in 2013. HRAs are intended to help you understand and improve your health! • {Biometric Screenings include: Blood Pressure; Body Mass Index; Cholesterol Levels and Glucose Levels} • Prior authorization required for high-tech radiology (e.g., CT scans, MRIs) and lower back surgery. • It is strongly recommendedthat you select a Primary Care Physician (PCP) for you and your dependents. • Thirty days of inpatient hospice care added.
New Level 4 Prescription Copay Current Prescription Copay Levels: 1) $5.00 2) $15.00 3) $35.00 New $50 Level 4 Copay • Applies to certain specialty drugs that require special storage and handling (list of drugs on Annual Benefit Enrollment website). • Will have a separate out-of-pocket maximum • If employee uses Diplomat Specialty Pharmacy the copay will be reduced to $15.00. • This will not affect many employees Call Navitus at 1-877-651-4943 or see Diplomat’s website at www.diplomatpharmacy.com with questions.
Other Health Insurance Changes • Be aware of dental changes under the following health plans: Anthem Blue, Network and Security. Dental is not mandated by Uniform Benefits and varies by plan. • Many health plans are making changes to their provider network so you should confirm that the doctors you use will still be available under your health plan in 2013.
Health Insurance Changes due to PPACA The Patient Protection and Affordable Care Act (PPACA) impacts benefits provided by State Group Health Insurance. Additional preventive services* for women will be provided at no cost (no co-insurance or copay) beginning January 1, 2013. A partial list includes: • Covered contraception • Well-woman visits if under age 65 • Domestic violence counseling The federal list of preventive services* that are allowed at 100% coverage has been updated. *IMPORTANT NOTE: The coding that the provider uses to bill for the service impacts whether or not the member pays coinsurance.
Dental & Vision Insurance Anthem DentalBlue will no longer be available in 2013 and coverage for current members will end on December 31, 2012. If you enroll in Dental Wisconsin for 2013, waiting periods for benefits are waived. Dental Wisconsin and VSP Vision Insurance are both offering an open enrollment period this year. Your health insurance plan provides coverage for an annual vision exam (except Standard Plan) and some dental insurance (except Standard Plan and SMP). Once enrolled in Dental WI or VSP, must remain in the plan for the entire calendar year.
Dental Wisconsin Select Plan & PPO Plan Administered by EPIC Life Insurance Company Provides partial dental coverage beyond what may be covered by health plan. Choose between one of two plans – the Select Plan or the PPO Plan. Select Plan does not provide preventive or diagnostic coverage. Both plans allow you to choose your provider but if you use a Delta Dental Premier Provider, you will have lower out-of pocket costs. This is an additional feature of Dental WI and is not a vision insurance plan.
VSP Vision Insurance Provides partial coverage for a vision exam plus eyeglass lenses every calendar year and eyeglass frames once every other calendar year. or Contact lenses every calendar year instead of eyeglasses or eyeglass lenses. Discounts on laser vision correction. See the provider of your choice but your out-of-pocket costs will be lower if you use a VSP provider. Your needs. Your benefits. EPIC Benefits+ EPIC Benefits+ is not offering an opportunity to enroll at this time. EPIC Benefits+ provides partial coverage for major dental services, a Hospital/Surgery benefit, a vision discount program and AD&D coverage. Provides vision insurance for additional cost. If you currently have EPIC Benefits+ vision insurance, you may cancel this coverage for 2013 – if you cancel vision insurance, you may not be able to re-enroll in the future. May remove dependents or cancel your coverage.

Canceling Dental or Vision for 2013 To cancel VSP, Dental Wisconsin or EPIC Benefits+ for 2013, you must submit an application to Human Resources, 203 Administration, no later than 4:30 p.m. on Friday, November 30th, 2012. Employee Reimbursement Account (ERA) • Flexible Spending Account (FSA) • Health Care FSA – use for eligible expenses not covered by insurance • Dependent Care FSA – use for dependent care expenses (child or adult daycare, preschool, after school care…) • Contribution is deducted from each paycheck before Federal, State and FICA taxes are calculated so you save money on taxes. • All benefit-eligible employees may participate in the ERA program except LTE, Fellow, Scholar, Graduate Intern/Trainee, or Post-Doctoral Fellow/Trainee.

ERA – Important Reminders You must enroll every year if you plan to use the program. If you don’t use it (your election) you lose it. Grace period for 2013 funds ends on March 15, 2014. WageWorksVisa® Cards will be reloaded with 2013 balances. First time enrollees will receive a WageWorks Visa® Card to use for Medical Account. If you are in the plan this year, have until March 15, 2013 to incur expenses and until April 15, 2013 to submit claims. • Try the new WageWorksEZ Receipts™ mobile application. Access your accounts and submit claims directly from your phone!
Individual & Family Life Insurance Annual Increase Option If you have I&F coverage effective on or before October 1, 2012 may increase coverage by the following amounts: Employee: $5,000; $10,000; $15,000 or $20,000 (Maximum coverage: $200,000) Spouse/domestic partner coverage: $5,000 or $10,000 (Maximum coverage: $100,000) Child(ren) coverage: $2,500 (Maximum coverage: $10,000) Spouse/DP and child coverage can’t exceed employee coverage level. Use Annual Increase Option form to increase coverage Can decrease coverage at any time by using regular application. You can use the Individual & Family Life calculator to see your current coverage and determine your premium for 2013. Calculator available at www.uwservice.wisc.edu/2013.
Your needs. Your benefits. The Wisconsin Retirement System (WRS)2013 Contribution Rates The WRS employee contribution rate for 2013 will increase from 5.9% to 6.65% for all employees except those covered by the Executive WRS category. This change will occur on the first check payable in 2013.
Saving for Retirement and Tax Sheltered Annuity Program (403b) Wisconsin Deferred Compensation (457) • Allow you to invest money on a pre-tax or post-tax (Roth) basis to save money for retirement. • Are voluntary and there is no employer contribution match. • You may contribute to both plans and you may • contribute on both a pre-tax and post-tax basis. • May enroll at any time with no start up fees • or minimum balances.
TSA & WDC Wisconsin Deferred Compensation (457) • 2012 Contribution Maximums** • Under age 50 - up to $17,000 annually under each plan • Age 50 or over - up to $22,500 annually under each plan • $8 (bi-weekly) or $20 (monthly) per pay period. • Annual Fee $15.00 per year • Enroll on-line at: www.wisconsin.edu/hr/benefits/retsav/tsaenroll.htm • No minimum contribution • Annual Fee $0 - $66/year* • Enroll or make changes at: www.wdc457.org/ *Depends on account balance Tax Sheltered Annuity Program (403b) • **The 2013 maximums will be released later this year.
