HSE LEARNING. INCIDENT: Wire-line Assistant was alighting from the Wire-line unit via a set of steps when he lost his footing on the second step from the top and fell forward onto the ground and broke his wrist. CAUSES : Failed to use / install hand rails Lack of due care and attention

HSE LEARNING • INCIDENT: • Wire-line Assistant was alighting from the Wire-line unit via a set of steps when he lost his footing on the second step from the top and fell forward onto the ground and broke his wrist. • CAUSES : • Failed to use / install hand rails • Lack of due care and attention • Lack of supervision (senior operator on site did not identify hazardous condition) • Hand rails went “missing” and unreported through department reporting process • Failure to STOP THE JOB • LEARNINGS: • Unit checklist does not have specific check point on steps / hand rails • Weekly unit condition list does not have specific check point on steps / hand rails Key Learning: Always ensure hand rails are in place before using steps Broken Wrist Use this Alert: Discuss in Tool Box Talks and HSE Meetings Distribute to contractors Post on HSE Notice Boards Include in site HSE Induction Contact: MSE511 for further information or visit the HSE Website Alert No 88 LTI # 7 July, 2012
