Overarching Evaluation To evaluate the development, delivery and outcomes of the chronic conditions management demonstrator programme (2008-2011) CCM Demonstrators – Deliverables, Evidence and Mainstreaming National CCM Cop April 22nd 2010 Roger Richards, Mark Kingston Aims To describe and assess the programme purpose and design To compare and contrast the differing approaches employed by the three demonstrator sites To assess effectiveness of the administrativeframeworkof the Programme To assess the impact of the demonstrators through specific outputs and outcomes To evaluate the awareness and delivery of the programme including the active participation of partners (including service users) in implementation. To determine the extent to which learning from the demonstrators has informed and supported CCM developments across Wales To identify individual and collective learning and best practice and to make recommendations based on this analysis for achieving efficient and effective use of resources

  18. Needs Analysis • Exercise that looks at the take up of guidance, information and learning from the three CCM Demonstrators sites. Incorporating two/three survey periods between 2010 and 2012, this work will contribute to an understanding of the impact of the CCM Demonstrator programme re providing advanced learning to other areas of Wales. • To assess awareness of the work of the CCM Demonstrators • To clarify areas of particular interest within health board’s/communities • Communications preferences of CCM community • To inform potential CCM Demonstrator roadshows and other events • To determine take up of learning from the CCM Demonstrators • To provide case studies of how learning has been used to inform CCM work • Sample • Directors of Community Primary Care • CCM Leads, LSB? • All Wales CCM community CCM Demonstrators – Deliverables, Evidence and Mainstreaming National CCM Cop April 22nd 2010 Roger Richards, Mark Kingston
