Portland Area – Demonstrations (15 tribes) 8 grants • Healthy Heart Project • Yakama • Diabetes Prevention Projects • 3 consortiums • Southern Oregon (Klamath, Cow Creek, Coquille) • NW WA Health Board (Lummi, Swinomish, Samish, Nooksack, Upper Skagit) • SPIPA (Skokomish, Squaxin Island, Chehalis, Shoalwater Bay) • Warm Springs and Colville

  5. Warm Springs (Healthy Heart) • 2nd annual diabetes awareness day • 20th annual heart smart dinner • Holiday and the Pi-ume-she Parade • Monthly fun runs/walks for community • Bunny run – Head start wee-bee runs • Community garden • Volunteers maintain • Elder drop offs

Nooksack Diabetes Health Fair • 17 booths/stations Foot exams, shoe fitting, nutrition and traditional foods, blood sugar and blood pressure testing, water challenge, eye care, behavioral health • Key note – Taking Control of your Diabetes" • Healthy lunch • Tools for diabetes management Log books, nail clippers, diabetes bracelets, glucose tablets, living with diabetes dvd, and low blood sugar treatment

  7. Samish 7th annual • Diabetes healthy heart project Key note: Frank Atherton Health exhibitors Comprehensive screening Massages Medical team for questions Educational resources and assistance 125 tribal members attended

  8. Klamath • Klamath Walk • Theme – past present and future: make the right healthy choices • Nurses station with glucose and blood pressure: before and after walk • Healthy Snacks • 117 tribal participants

  9. Cowlitz • DPP program 25 participants 16 week class All have been successful in lifestyle changes for health • 9thAnnual Tribal Health Walk Screenings Health education and awareness 450 participants

  10. Cow Creek • I strengthen my nation Youth conference focus on nutrition and physical activity • Monthly support group • Monthly clinic • Transformation café Focus on weight loss

Umatilla Activities Public outreach 3 open mic ( 2 attendees – one radio show interview) Diabetes survey at fun run 65 surveys for diabetes outreach and activity Nurses diabetes trainings • 6 trainings – 30 attendees • Various topics Community outings • Family snow showing • Head start gardening – cooking class with veggies Community screenings • 4 senior center – 30 screened • Family fun night – 20 screened – 150 attended • Employee screening at Casino – 16 screened

Nez Perce - Women's health day • 16 health display booths • Nutrition, traditional food, living an active life style, and tobacco cessation • Key note – diabetes prevention • changes in diet and exercise for diabetes control • Key Note – living smart • Living tobacco free and behavioral risk factors

  13. SDPI programs in NW • IHS diabetes audit – due on May 15, 2013 • Portland Area Tribes had 100% audit submission • Health Status Report – based on the audit; every tribe receives a report that shows a comparison (to Portland Area) and a trend (of their program)
