An individual can work with a voluntary provider to design an innovative and individualized program to meet his/her specific service and support needs
On behalf of that individual, the agency develops a proposal specific to the applicants interest
Once the plan is approved, OMRDD contracts with the agency to carry out the services and supports for the individual as specified in the proposal

7. Transition Planning with Consolidated Supports and Services (CSS) Getting Help: What is a Start-up Broker?
Families and MSCs often dont have the time or ability to do all the tasks associated with getting a SD/CSS application and plan completed.
Startup Brokers are independent Contractor hired by the person for a negotiated price based on the persons needs (100% state funded)
Assists the person to develop the Self-Determination/Consolidated Supports and Services (CSS) application and initial SD/CSS service plan
Tasks are separate from those of your Medicaid Service Coordinator
