Spring 2014 – What should individuals/families do then? • Complete Developmental Disabilities Resource Tool (DDRT) • Submit the Support Coordination Agency Selection Form (60 days prior to graduation) • Develop ISP (assignment of SCA, planning team meets, SC writes plan, completed & approved within 30 days of SCA assignment) DHS/DDD
