2. EXIT CONFERENCE

3. OBJECTIVE OF AN EXIT CONFERENCE • TO INFORM THE FACILITY OF THE SURVEY TEAMS PRELIMINARY FINDINGS AND OBSERVATIONS

4. EXIT CONFERENCE The following persons will be invited to the exit conference: Ombudsman, an officer of an organized resident group (if one exists), one or two residents, and staff as invited by the facility administrator.

5. EXIT CONFERENCE • An explanation of the preliminary findings will be given. • Identifier numbers of residents driving Immediate Jeopardy deficiencies will also be identified as well as a complete sample list with identifier numbers will be left at the facility.

6. EXIT CONFERENCE • No specific Tag numbers will be given only identified problems such as: falls, pressure ulcers, resident to resident altercations without interventions etc. Problems identified as Immediate Jeopardy areas will be pointed out again during the exit conference.

7. EXIT CONFERENCE • The findings are subject to QA review both by the State Survey Agency and CMS. In addition to tags related to the areas of Immediate Jeopardy given at the exit conference, others tags might be added during the QA reviews. Examples of which are:

8. EXIT CONFERENCE • There should be few instances where the facility is not aware of surveyor concern or has not had an opportunity to present additional information prior to the exit conference. On these rare instances the facility will be allowed to present the additional information at the exit conference.
