  2. Congregational Health Network Results • 50% Decrease in mortality • 20% Decrease in readmissions • $4 Million in savings

Challenges in Ector County • Leads the State of Texas in the top 10 leading causes of death • Heart Disease • Cancer: Colorectal, Breast, Lung • Chronic Lower Respiratory Disease • Accidents • Cerebrovascular Disease • Alzheimer’s Disease • Diabetes Mellitus • Influenza/Pneumonia • Chronic Liver Disease/Cirrhosis • Septicemia

Why Become A Member of the Faith and Health Network… • Should you be admitted to Medical Center Hospital, at your request we can assist you in notifying your congregation • If you need help with any resources, you have a contact person to support you • You will receive health & wellness information and community resource information on a regular basis • You are helping to promote wellness in your community as you also continue to share your faith with others • You will receive a folder with helpful information to get you started • With your membership card, you can receive benefits provided to you by participating local businesses • NO COST TO YOU
