Five Rivers Health Center serves as a medical home for the low-income patients of Dayton, Ohio by providing both primary and specialty care services to patients through the academic rotations for the Wright State University Boonshoft School of Medicine residency and fellowship programs FRHC consists of three (3) former hospital residency clinic departments - Family Health Center (2345 Philadelphia Drive), Medical Surgical Health Center (725 S. Ludlow), and the Center for Women's Health (Wyoming & Brown Streets) at five (5) locations In total, there are 92 attending physicians, 5 Certified Nurse Midwives, 190 residents, 8 fellows, and 115 employees
Services Include Primary Care – includes Family Medicine, Internal Medicine, and OB/Gyn Specialty Care— includes High Risk Pregnancies, Orthopedic-Non-Surgical, Infectious Disease, Hematology/Oncology, Rheumatology, Renal, Mental Health, Neurology, Adult Transitional Sickle Cell, and Healthy Lifestyle Clinics • Plans for the Near Future • Wright Health Pediatrics will move to FRHC at the Center for Women’s Health Location on January 7th • Family Health Center will relocation and expand from 16 to 39 exam rooms in the old Elder Berman store in the early summer, 2013 • Will apply to become a FQHC
