Develop a Plan • Define your community (regional, county, etc.) • Identify the team • Define the process • Models • Assessment methods • Select indicators of interest • Consider appropriate comparisons • Identify data sources or collection methods • Methods of community engagement • Prioritization methods

  3. Conduct the Assessment • Gather or collect the data • Assemble data in ways that are easier to analyze and interpret • Tables • Charts & graphs • Visual depictions (photo journals, etc.) • Compile a written report (data profile) • Share the report with broader community • Solicit community feedback

  4. Identify Priorities • Analyze the findings & community input • Identify priorities • High volume • High risk • Problem-prone • Actionable

  5. Develop CHIP • Drill down, identify root causes • Identify improvement strategies • Assign specific responsibilities, timelines • Create a written report • Disseminate report to community

  6. Implement the CHIP • Implement intervention strategies • Continue to monitor and assess • Adjust intervention strategies as needed • Incorporate into your strategic plan

  8. Key Points • Think of health broadly • Involve community partners • View as a management dashboard • Iterative - not a one-time process • It’s not rocket science!
