  3. Family Story • What has the family learned? • What new skills have been built? • How have they changed? (this could include TPR, adoption, guardianship, etc.) • How have they grown? • What has their experience been like? • How is their family stronger? • Describe the benefits of collaboration. • Describe where the family is now compared to then. • How has the family dynamics changed. • How skills/techniques/time lines has resulted in positive growth. • Going forward story.

Strengths • New skills learned. • Resiliency. • Doing what is necessary. • Case progress. • Personal attributes that helped them succeed. • Coping skills. • Client present and cooperative. • Success of family. • Developing of community resources. • What have they learned through the process. • Changed behaviors. • Support system in place.

  5. Needs • Focus on community resources. • Are they prepared to continue success without DFCS services? • Can they identify and rectify issues on their own? • Supports that may be needed. • Plan of action. • Long range goals. • Maintaining relationships. • Relapse plan. • Any pending needs.

Family PLan • Personalized goals. • Moving? • Buying new home? • Going back to school? • Where do they see themselves one year from now? • Discharge Plan. • Community Resources. • Family Support. • How to continue to utilize skills and knowledge obtained through services. • After care plan. • How to become/remain self-sufficient. • Transitional plan to reunify children.

  7. Time frame of closure FTM • At least 2 weeks prior to closing case. • Only if sure the case will be closed.

  8. Who to invite to closure ftm • Community resources. • Case manager. • OFI. • Providers related to the case. • Children. • Family.
