Project Description • Consultant to Provide Professional Services in material testing and Construction Inspection as described in the RFQ. • Consultant to Provide Professional Surveying Services as described in the RFQ. • Consultant to act as an extension to the Engineering Staff of the Department of Port Control. • Testing and Inspection Consultant shall be certified with accredited agencies listed in the RFQ, under the Scope of Work, Section 2.3. • Testing and Inspection Consultant must have a local lab and testing facility
Project Description • Consultant shall be certified to practice in the State of Ohio. • Testing and Inspection Consultant shall be certified to perform Asbestos sampling, and act as third party for Asbestos Oversight and Air Monitoring support. • Consultant shall be adequately staffed for quick responses to the needs of the Department of Port Control, and provide deliverables on a timely basis in accordance with the RFQ and contract requirements. • Testing and Inspection Consultant shall be certified with the City of Cleveland, Building and Housing, to act as a special inspector under the requirements of Chapter 17 of the Ohio Building Code.
Scope of Work • The Testing and Inspection Consultant shall: • Provide testing and inspection services, as required. • Review, approve, and perform mix designs. • Report on testing and inspection work performed. • Provide recommendations on geotechnical investigations and Quality Assurance . • Sampling of material for testing, including Asbestos and other environmental sampling and monitoring tests. • Participate in meetings and report issues and findings to the Airport Engineer.
Scope of Work • The Surveying Consultant shall: • Provide professional surveying services, as required. • Locate structures and utilities. • Shoot spot and topographical elevations. • Perform Airport Obstruction Surveys as required per FAA circular for certification maintenance. • Report on surveys performed using both electronic files and various sizes of hard copies. • Participate in meetings, and report issues and findings to Airport Engineer.

Contract Clause Inclusion Assurance 49 CFR §26.13: Applied to: Contract with Prime Contractor and all sub-agreements with any subcontractor/supplier or lower tier subcontractor/supplier whether DBE, Non DBE or SBE. “The contractor, sub recipient or subcontractor shall not discriminate on the basis of race, color, national origin, or sex in the performance of this contract. The contractor shall carry out applicable requirements of 49 CFR Part 26 in the award and administration of DOT assisted contracts. Failure by the contractor to carry out these requirements is a material breach of contract, which may result in the termination of this contract or such other remedy, as the recipient deems appropriate, which may include, but not limited to withholding monthly progress payments, assessing sanctions, liquidated damages, disqualifying the contractor from future bidding as non-responsible.”
