Live? • What ‘quality of life’ considerations will be most important to you? (Spending time with family and friends, moving to a certain city or town, sleeping soundly each night with the knowledge you are financially secure, having bills paid off, etc) • Will you continue to work part time? • Will you consider moving to a smaller house, apartment or condo? • What leisure activities do you look forward to? (Hobbies, travel, sports, volunteering, involvement in civic or church-related activities, recreation, taking classes, etc.) • What are your lifelong goals? What are the things you hope to accomplish? • Who will depend on you financially? • If you were to retire tomorrow, what percentage of your annual income do you think you would need to live on? (e.g., 70%, 80%, 90%, 100% ??) • What future financial needs will you encounter? (Health insurance costs, planning for Long Term Care needs, tax and estate planning considerations, gifting to your children or grandchildren, etc). • When you think of your ideal retirement, do any other thoughts or aspirations come to mind?

(If you have not logged in before, your initial login is generally your SSN plus your Date of Birth, but must be changed upon first access to the site). Follow the instructions on the page to log in, and if you have any problems, please call our Call Center at 866-401-5272 (option 3, option 3). • Determine what percentage of your account is invested in: • U.S. stock funds: ________% • Foreign stock funds: ________% • Bond funds: ________% • Cash / stable value: ________% For additional assistance on the subject of investing, including sample investor portfolios, please see the ‘What mix of funds is right for you?’ worksheet within the participant website. If you feel any investment changes are warranted for your account, please make those account realignments through the participant website.Note: For fact sheets, prospectuses and additional information on your fund lineup, please see the participant website. • What percent of pay are you currently contributing to the plan? Enter here: _________% • In the participant website, select the Resource Center tab, then select Retirement Financial Calculators. Run an analysis of your financial situation using the Savings Calculator and the Retirement Shortfall Calculator. • Are you saving enough in the 401(k) Plan to reach a successful retirement? Yes / No (circle one)How far off track do you feel you are? • If you answered “No” to question #5, what percent of pay would you need to save to fill the gap? _______% of pay • Can you increase your deferral rate now? What new contribution rate could you live with? _______% • Check this box after you have submitted your deferral rate change form to Human Resources (plans using online enrollment can make this change in the participant website). • If you have experienced any life changes which require an updated beneficiary form, check this box after you have updated your beneficiary form with Human Resources. • It’s important to revisit these issues annually and make any needed adjustments.
