National Vision for ADRCs “To have Aging and Disability Resource Centers in every community serving as highly visible and trusted places where people of all incomes and ages can turn for information on the full range of long-term support options and a single point of entry for access to public long-term support programs and benefits.”

  3. PA Vision for ADRCs “To improve access to long-term care services through an integrated network of partners committed to expand the use of community based solutions, promote consumer directed decision making, and enhance the quality of services.”

ADRC GOALS • Statewide expansion of ADRC presence • Information, Referral and Awareness\Full Range of Long-Term Services & Supports • Options Counseling and Assistance\Decision Support • Streamlined Eligibility Determination for Public Programs • Transition Support and Services • Partnerships and Stakeholder Involvement • Quality Assurance and Continuous Improvement

ADRC GOALS • Coordination of Aging and Disability service systems • Individuals of all income levels are served • Management information systems that facilitate service delivery, supports integration of data, streamlines access to information and options • Comprehensive statewide web resource database – searchable & accessible

ADRC OUTCOMES Serve as “no wrong door” access points at the community level to individuals needing public benefits, as well as counseling and advocacy to overcome barriers to using benefits and returning or remaining in the community Minimize confusion Improve coordination Simplify access Enhance individual choice Promote informed decision-making Identification of gaps in services
