Provider Registration Process Steps • Agency will receive an Approval Letter from DIDD Provider Enrollment Coordinator. • Business Services will enter the DIDD Referral information into the TennCare Provider Data Management System (PDMS). • The Agency will then receive an email titled “DIDD Referral” from Provider.Registration@tn.gov that will include a link to the PDMS portal and instructions on how to register. Provider Registration Process Steps (cont.) • The Provider Agreement is created during the agency’s registration process. • Once the Agency has signed the PDMS contract and submitted to TennCare then DIDD will review/approve and sign the contract. If there is any other information or documentation needed the agency’s portal account will be returned to them with instructions on what is needed. • Once DIDD has reviewed/approved and signed the contract it will be forwarded to TennCare for review/approval and final signature.
Provider Registration Process Steps (cont.) • The Agency needs to watch for any emails from Provider.Registration@tn.gov that will be titled “Action Required” and complete the required actions as soon as possible. The Agency also must check their PDMS account frequently and once all signatures have been obtain on their contract they need to inform DIDD by contacting Julia Jinnette at Julia.Jinnette@tn.gov • At this point DIDD will set up Billing Codes for this Agency and inform the Regional Office. • The Regional Office will contact the Agency to inform them of the next step as a DIDD Provider.
Forms and Documents that MUST be upload into the PDMS portal. • A copy of each applicable Business License (when applicable) • A copy of each DIDD Licenses and/or DoH’s PSS Licenses (when applicable) • A copy of the provider’s Proof of Insurance. • A copy of a Voided Check or Bank Letter – to be uploaded on the Licenses & Classifications section in the PDMS • A Completed W-9 form (link to this form is in the portal on the Substitute W9 Form section) • A completed Crosswalk form (if applicable – see instructions on Substitute W9 Form section in the PDMS) • The provider should be prepared to provide other types of documentation upon TennCare’s request.
New Provider Agreement Required When Following Occurs: CHANGE IN OWNERSHIP/LEGAL STRUCTURE: The DIDD Provider Manual Section 5.3 states that a new application must be submitted any time a change in ownership occurs. The purchasing agency may not bill DIDD for services rendered until a new provider agreement has been established and signed by DIDD, TennCare, and the new agency. An agency may not continue to bill DIDD using the previous agency's billing numbers and site codes once a change has occurred. Services may not be billed retroactively, should an agency provide services before a new provider agreement has been established. It is imperative that a new application be submitted as soon as a decision to change ownership has been determined, as the application process may take several or weeks, or months. For agencies licensed to provide Professional Support Services through the Department of Health (DOH), Rule 1200-8-34-.03 states that, a proposed change of ownership must be reported to the department [DOH] a minimum of thirty (30) days prior to the change. A new application and fee must be received by the department before the license may be issued. Changes in Agency Contact Information Changes in Ownership February 15, 2007 Page 2 of 2 Circumstances constituting a change in ownership may include, but are not limited to: • Partnership changes • Corporation changes • Leasing Changes • Transfers (Please refer to 1200-8-8-.02(e) and 1200-8-34-.03(b)). Other agencies licensed through DOH and/or licensed through the Department of Mental Health and Developmental Disabilities (DMHDD) are also required to submit a new application and fee before a change in ownership can occur. (See Rules 0940-5-2-.09 and 1200-8-8-.02(d)).
