The Case for Change The current health & social care system is not fit for purpose Why? • Set up for 5 day working for patients with a single condition • Responsibility taken away from people for their own care • People are living longer (good) but their needs are greater • More people with long term conditions and complex needs • Fewer people have close family support – need a new model • Poor outcomes and large health inequalities • Financial pressure to deliver more for less Richard Jones – 7 May 2014 | Third Sector Lancashire
