  2. 2013 OPEN ENROLLMENT Welcome to the 2013 FCER Open Enrollment! Today we will review: • Medical Plan Options • Dental Plan • Vision • Life, Short Term and Long Term Disability • Flexible Spending Account

  3. 2013 OPEN ENROLLMENT You, your legal spouse and children (under the age 26) are eligible to be enrolled in our plan During Open Enrollment you may: • Enroll or Change the Medical Plan you and your eligible dependents are enrolled in • Enroll or change your enrollment in the Dental Plan • Enroll or change your enrollment in the Vision Plan • Increase your Voluntary Life benefit – may be subject to medical underwriting • Enroll in the Flexible Spending Account • Drop coverage All changes requested will be effective January 1, 2013

  4. NEW MEDICAL PLANS • January 1, 2013 our Medical Plans will be provided through Cigna • You may choose from two plans: • Base HSA Plan • Buy Up PPO Plan

MEDICAL PLAN CHOICES Two Medical Plans Choices: • HSA Qualified Plan • Lower Premiums • Higher Out of Pocket • May be eligible to contribute to a tax qualified HSA Account • Buy Up PPO Plan • Higher Premiums • Lower Out of Pocket Costs (for most services) ** Both plans use the Cigna OAP Plus National Network of Provider

MEDICAL PLAN CHOICES HSA Qualified Plan: • Is a high deductible health plan • Calendar year deductible must be met before the plan will pay any benefit Services Include: doctor office visits, urgent care, hospitals, surgeries and prescription drugs • Annual in-network deductible: • $3,000 for single/$6,000 aggregate for a family • Once your deductible is met the plan pays 100% for all eligible in-network services • Annual out-of network deductible: • $6,000 for single/$12,000 for a family • The plan will then pay 70% of eligible charges • Preventive care is covered at 100% and the deductible is waived

MEDICAL PLAN CHOICES If you enroll in the (Base) HSA Qualified Plan: • You may be eligible to participate in a Health Savings Account (HSA) • Deposits made in an HSA Bank Account • up to $3,250 for a single and $6,450 for a family • Deposits made through payroll deduction • pre-taxthrough ACS|BNY Mellon HSA Solutionor you may make a post tax contribution and take a top line deduction on your income tax filing • Contributing to an HSA you MAY NOT participate in a Medical Flexible Spending Account (FSA) – not even through your spouse’s employer • You MAY contribute to a limited-FSAfor your dental and vision expenses • Use your HSA to pay forqualified medical expenses

MEDICAL PLAN CHOICES Enrolling in the (Buy Up) PPO: • Higher premium but lower out of pocket costs • Primary Care Office Visit = $25 • Specialist Office Visit = $50 • Urgent Care = $25 • Prescription Drugs = $20 Generic | $35 Brand Name | $50 Non Preferred Brand Name • All services outside the doctors office • Are subject to the calendar year deductible • $1,000 individual or $3,000 family • After the deductible has been met the plan pays 80% for eligible in network services • Preventive Care is covered at 100% - deductible and copayment waived ** Both plans use the Cigna OAP Plus National Network of Provider

ONLINE TOOLS AND SUPPORT Cigna offers a full suite of on-line tools to support you and your family’s healththrough www.mycignaforhealth.com • Online Access: • Register as a new Cigna member • Print a temporary ID card or order a New ID Card for you and your covered dependents • Check the status of a claim • Find a provider • Print Forms • Estimate the cost of health care services • Find information on health conditions • Complete Wellness tools and resources • Check your HSA Balance • Find discounts on health and beauty items

DENTAL PLAN Our Voluntary Dental Plan is offered through Delta Dental • Freedom to visit any licensed dentist in the world • Lower out-of-pocket costs when visiting an in-network PPO dentist • Guaranteed coinsurance amounts and no balance billing from any Delta Dental dentist • Visiting a non-Delta Dental dentist, will increase your costs • No waiting periods

  18. LIFE SHORT TERM DISABILITY LONG TERM DISABILITY:THE HARTFORD

BASIC LIFE INSURANCE • FCER provides a life insurance benefit of 1.5 x’s your annual salary • The Basic Life Insurance includes an Accidental Death and Dismemberment benefit that doubles your life benefit if your death is the result of an accident • This benefit is provided at “no cost” to you

SHORT TERM DISABILITY STD Coverage • Provides a benefit should you become too sick or injured to work for a short period of time • Will pay you 60% of your weekly income after you’ve been disabled for 7 calendar days • The policy will not cover disabilities caused by on the job injuries or illnesses • The plan will pay a benefit as long as you are disabled up to 25 weeks total • This benefit will be provided at “no cost” to you

LONG TERM DISABILITY Long Term Disability or LTD • Provided to all full time eligible employees of FCER at “no cost” to you • The plan will pay a benefit of 60% of your income every month that you are disabled after a 26 week elimination period • The plan will pay up to your normal social security retirement age

FLEXIBLE SPENDING A Flexible Spending Account is a method for employees to save tax dollars on everyday expenses that you and your family experience • Out of pocket expenses include: • Health, dental, vision & prescription expenses • Day care expenses for your children (under the age of 13) • You may elect up to: • $2,500 for medical expenses annually • $5,000 for day care expenses annually • The amount you elect: • Will be withdrawn from your paycheck on a pre-tax basis in equal installments • The full amount you elect for your medical spending account will be available immediately for your use on any eligible out of pocket expenses • Day Care Expense Account is Cash In / Cash Out

FLEXIBLE SPENDING ACCOUNT • You can use your TaxSaver Plan Debit Card to pay for your out of pocket expenses • You must save all receipts for items purchased with your Debit Card • The IRS requires that TaxSaver substantiates a minimum percentage of all transactions to be in compliance with regulations • You may also request reimbursement by submitting a claim via mail, fax or e-mail • Information found on www.taxsaverplan.com • Claim Forms • A tax savings calculation tool (use before you enroll) • Reimbursements are processed every Friday and can be paid by check or direct deposit
