Primary prevention/promoting wellbeing: aimed at people who have no particular social care needs or symptoms of illness focus is on maintaining independence, good health, and promoting wellbeing Secondary prevention/early intervention: aims to identify people at risk and to halt or slow down any deterioration actively seek to improve a person’s situation. Tertiary prevention: aimed at minimising disability or deterioration from established health conditions or complex social care needs. What do we mean by prevention?

Delaying and reducing the need for care and support Shifting the model from ‘doing to or for’ to enabling people to ‘do for themselves’ before a time of crisis or significant deterioration in ability Seeking to protect our most vulnerable residents and helping people to lead independent and fulfilling lives giving them more choice and control over their care and support Supporting and maintaining residents ability to engage positively in their own communities Enabling residents to manage their own health and social care needs Why commission for prevention?
