Principle Functions of CG-CVC-4 Mariner Credentialing Program Policy Division • Supervision and overall management of the credentialing program • Interpret standards and regulations • Serve as the technical control office for NMC • Develop program policy

Current MCP Initiatives/Projects • Merchant Mariner Medical Advisory Committee (MMMAC) • Implementing Section 809 of the Coast Guard Authorization Act of 2010 – TWIC • 2010 Manila Amendments to STCW • 37 NVIC Projects, including new and revision • Appeals
MMMAC • Committee’s current tasks: • Revision of NVIC 04-08, Medical and Physical Guidelines for Merchant Mariner Credentials; • Top medical conditions leading to denial of mariner credentials; • Revising the CG-719K Medical Evaluation Report Form; • Creating medical expert panels for the top medical conditions; and • Developing Designated Medical Examiner Program. • Next meeting, September 25th – 26th at Piney Point
Section 809 of the 2010 USCG Authorization Act – TWIC • Evaluate the implementation of changes to the TWIC requirements as mandated by the Coast Guard Authorization Act of 2010, Section 809
2010 Manila Amendments to STCW • Update standards of competence required for emerging technology; • Introduce new training and certification requirements and methodologies; • Improve mechanisms for enforcement of the Convention’s provisions; and, • Detail requirements on hours of work/rest, prevention of drug/alcohol abuse, and medical fitness
NVIC/Policy Projects • Ensure alignment with STCW revisions (i.e. Company roles & responsibilities, RFPNW, RFPEW, OICNW, OICEW, etc.) • Grandfathering • Tankerman • Medical • OSV • Marine Safety Manual, Volume III • Course approvals

Current Federal Register Notices: Request for Public Comments • Medical Waivers for Applicants with Anti-Tachycardia Devices or Implantable Cardioverter Defibrillators (ICD) • Medical Waivers for Applicants with a History of Seizure Disorders
ICD Waiver/Seizure Disorder: Background • Per 46 CFR 10.215, CG may issue medical waivers • NVIC 04-08 states ICDs are generally not waiverable & mariners with seizure disorder should contact NMC for guidance • Problem: NVIC 04-08 does not identify waiver criteria associated with ICDs or seizure disorder. Difficult for CG to evaluate/assess whether applicant’s condition warrants waiver.
ICD Waiver: CG Request for Public Comments • Likelihood of an ICD inappropriately firing and whether shock could incapacitate mariner • 12 questions used as criteria for granting a medical waiver • Full list located in FRN
Seizure Disorder Waiver: CG Request for Public Comments • Criteria associated with granting a waiver for seizure disorder • Consistent with Federal Motor Carrier Safety Administration (FMSCA) guidelines & recommendations of their Medical Expert Panel (MEP) • Full list located in FRN
