DIABETES SUPPLIES   (all prescriptions with 3-5 months refill, except for meter kit) INSULIN: Novolin or Humulin N, Humalog, Lantus, etc (2 vials) Disposable insulin pens: Novolin or Humulin brand (1x5) Cartridges 3ml (1x5) for insulin pens (Novo Junior or Huma- Luxura) with ½ unit increments.   Humalog Kwik or Novolog Flexpen disposable pens with 1 unit increments. DISPOSABLE PEN NEEDLES #100 INSULIN SYRINGES: ¼ cc, 1/3 cc, ½ cc, 1cc (needed amount for month) METER KIT #1 (specify brand. eg. One Touch Ultra) STRIPS #250 testing 8 times per day LANCETS #250 testing 8 times per day ALCOHOL SWAPS #250 8 times per day GLUCOSE TABLETS #50 3 tabs prn as directed URINE CHEMSTRIP K #100 prn as directed GLUCAGON EMERGENCY KIT #2 prn as directed

Insulin Pump Supplies • Syringe Reservoir 3ml 1 every 2 days #90 (this is a 6 months supply). • Infusion set 1 every 2 days #90 (must write diagnosis on prescription). • Humalog, Novolog, or Apidra insulin – please prescribe enough vials per month to accommodate daily insulin totals. • IV 3000 #100 as directed refills x5 • IV Prep pads #100 as directed refills x5 • Blood glucose test strips 8x/day #250 refills x5 • Lancets 8x/day #250 refills x5 • Alcohol swabs 8x/day #250 refills x5 Please note: In case of pump failure patient should have available at home Lantus insulin and insulin syringes (in addition to the usual Ketostix, Glucagon Emergency kit, glucose tablets)

  8. Hypoglycemia 15/15 rule 15 grams of carbs and recheck blood glucose in 15 minutes. Repeat treatment and recheck until blood glucose is more than 70
