Goal of Dissemination Activities • Achieve widespread national dissemination on the lessons learned from NC’s practice transformation initiatives • Advance state level collaborative initiatives in three states to strengthen the capacity of primary care practices to engage in continuous quality improvement.
Widespread Dissemination • Webcasts (like this one) • Issue Briefs • Lessons learned on establishment of NCHQA • Online compendium of tools to support this work • E.g., tools referenced by Denise Hewson on CCNC • Conference presentation at NASHP Annual State Health Policy Conference
Intensive Collaborative Activities with 3 States • Kick-off meeting in NC • States have preliminary implementation plans • Feedback from NASHP/NC faculty • Identify technical assistance needs, meet NASHP/NC contacts • More in-depth learning about NC infrastructure • Quarterly webinars on design/implementation issues • Mentoring calls among participants with similar responsibilities (e.g., Medicaid directors, AHECs, RECs, Academic Centers) • Listserv
States Receiving Assistance from IMPaCT • Oklahoma • Arkansas • Missouri • Colorado • New Mexico • Oregon • Kansas • Kentucky • Illinois • Pennsylvania • New York • New Jersey • Vermont
State Selection through Request for Application • To ensure that adoption states are committed and prepared • Conditions of participation • Workplan • Key member participation • Attendance at, and support for, NC site visit • Participation in technical assistance events
Key Selection Criteria • Multi-sector, state-level collaboration • Core team of at least 3 members • State health agency • Entity providing support to primary care practices for quality improvement and transformation • Demonstration of: • Plans to achieve practice transformation strategies • Expected benefits from Learning Community technical assistance
