Integration of Home Care within levels of health care; different sectors; governmental and non-governmental organizations and different branches of care. Purpose:To discuss opinions and experiences of the participants in terms of integrating home care in a country which is in a transformation periodof primary health care services and recently organized home care service delivery.
A newhealthdeliverysystem has beenintroduced in 2003 bytheTurkishMoH. • primaryandsecondarycarewerereorganized. • “homevisitorcalls” weredefinedto be deliveredbyprimaryhealthcareworkersandnurseswereallowedto deliver privatehomehealthservices, too. • hospitalbasedhomehealthserviceshavebeenstartedwithoutcreatingthenecessaryenvironment.
But, none of these met thecurrentdefinition of “homecare”. Services enablingpeopletoliving in theirhomeenvironment In 1996, privateorganizationsstartedto deliver homecareservices. • totallypaidbythepatients • no legal regulation at that time • theneedto service wasextremelygreat, privatesectorwassoonfollowedbylocaladministrationslimitedtodesign of theorganizations!!!
In 2005, withthehelp of theefforts of the service deliverersandnon-governmentalandacademicorganizationsworking in thearea, a by-lawwasannouncedbyMoHcalled “Regulation of Home Health Services”. In 2004, a fewgovernmentalhospitals, mostlylocated in smalltownsstartedto deliver homehealthservices. No paymentbytheSocial Security Systemduetolack of regulations.
“Regulation of Home Health Services DeliveredbyMinistry of Health” in 2010, andrapidlyorganizedhospitalbasedservicesacrossthe Country. • At least; thepaymentsystemwasdeclared!!! • A “windowdressing” activitywithoutany background organization. • Preventivehealthservices of homecare, primarylevelhealthservicesandfollow-up of chronicdiseases of home-boundpatients, wasdefined as duty of primarycareteam.
As a conclusion; • InTurkeytherearemanydifferentorganizations of homecare in thefield. • Socialhomecareservicesaredirectedby an othergovermentalorganization. • We as a countryhavemanyorganizationsthatallocatedresourcestobettermeettheneeds of thispopulation. • Lack of theexperience of carecoordinationforpeoplewithmultiplehealthandsocialneeds, • a methodical approach to deliver integratedservice, • humanresourcestrainedappropriateforthisissue, • standarts to deliver the homecareservice • to measure the outcomes while ensuring efficient access to the health care system.
Evidence suggests that Integrated Home Care, which could be defined as; “integrated pathway between hospital services, primary care and social services for patients withhome care” • What strategiescan be used to integrate home care services within different levels of care? • How could social and health aspects of home care be integrated? • What is the role of reimbursement system in integration of home care?
