  4. The Purpose of the Quality Improvement Team (QIT) • This QIT is created for the purpose of designing, promoting and supporting family services within your program. • Establish and empower appropriate staff to coordinate the efforts to get family services up and running

  5. Please share on where you are with your QIT • How did you organize the QIT? • How did you assign various responsibilities? (e.g., who will collect and maintain the data)

  6. Please share on where you are with your QIT • How does communication among QIT members happen? • Does your QIT have separate meetings? If so, how often? • If not, how do you communicate and make decisions? • What is discussed during the meetings? • How is feedback shared with staff?

  7. Would anyone like to share if you have not been successful with establishing a QIT? • Talk about what the challenges are that are inhibiting the successful launch of a QIT at your program. • Would anyone like to share on how you are addressing these challenges?
