Data Submission Compliance • Verify “PART” from the 1st to 10thof each month • Work on “Action List” twice a month – update facility electronic data systems • Submit 2728 form within 45 days from the date patient starts at the facility or from 1st to 10th of each month • Submit 2746 form within 30 days from the date of death or from 1st to 10th of each month • Verify or submit Clinical and Vascular Access data for each month by 25th of each month ew Statement of Work • Run reports to verify data submission – Audit forms report, missing clinical data, Notifications & Accretions report

  3. Yearly 2744 Submission

  4. 2744 Year End Survey • Submit 2744 forms by March 1st each year • Attestation by February 28th of each year • This activity has a serious payment reductions implications under pay for perfomance
