This document is designed to help you prepare for your presentation at the end of your safety improvement project What is expected of me? • Some of you will be asked to do a brief (under 10 minutes) presentation regarding your project • Others will be asked to display a poster presentation • We recommend that you limit your presentation to the format provided in this document • On the day you may also be asked some questions about your work What will I find in this document? • An example completed presentation • A blank template presentation Remember – keep your presentation simple!

EXAMPLE Safety improvement project aim EXAMPLE How many, by when, measured by? The aim of this project was to improve the reliability of prescription of low molecular weight heparin (LMWH) by introducing a ward round check list Before the test less than 80% of patients had LMWH prescribed when indicated (Review of 10 charts) The target was 100% of items on the checklist would be completed on one ward by the end of the project (within 8 weeks). The list of things to check includes the prescription of LMWH where indicated Measured by checking off list during ward rounds to ensure it was done

Check list was drafted • D – Check list introduced and tested on ward round for a week • S – Summarise what was learnt. Revisions needed to checklist as certain points were not clear • A – Check list was revised
EXAMPLE Completed PDSA cycle – number 2 • P – Revised checklist was discussed with mentor • D – New checklist was tested on ward round for a week • S – Completion of checklist increased to 80% – but in some cases there was confusion about who should be completing it • A – Responsibility for checklist should be assigned to someone
EXAMPLE Completed PDSA cycle – number 3 • P – Test idea that someone needs to be responsible for making sure the checklist is completed • D – Responsibility for checklist completion assigned to nurse-in-charge • S – Checklist completion increased to 100% • A – Aim achieved: next step may be to introduce to neighbouring ward

What did not? • Team excited about using check list • Checklist did not slow down the ward round • Didn’t check with ward manager before starting first test – which led to confusion • Checklist was initially not always used when I was absent • Sometimes we ran out of printouts
