Value Statement – Well-Being The public and private sector will work together to ensure that children in Georgia receive appropriate services to meet their educational and physical/mental health needs.
Goal 3 – Well-Being 1 Children in Contractor's care receive the prescribed preventive medical, dental and vision services. 15 % of total score – 15 points
Outcome Measure 1 Contractor will ensure that children in care receive required Georgia Health Check screenings per Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program standards, on schedule.
Definition and Calculation • Definition – Georgia Health Check Program periodic screenings standards • Calculation – Screenings completed on schedule/screenings due
Scoring • 100% of goal – 15 possible points
Goal 3 – Well-Being 2 Children in Contractor's care will receive prescribed educational services. 15 % of total score – 15 points
Outcome Measure 1 Contractor will ensure that all children in care are enrolled in a public school system, an accredited private school, a Georgia Department of Education/Local Education Agency approved residential facility school, or an approved educational plan within 2 days of placement.
Definition and Calculation • Definition – School must be accredited, approved or operated by local public school system • Calculation – Number of children enrolled in approved school or educational plan within two days/total number of children admitted during the contract year
Scoring • 66% of goal – 9 possible points
Outcome Measure 2 Contractor will ensure that children in their care have no more than 5 unexcused absences per school calendar year.
Definition and Calculation • Definition – Unexcused absences are defined by the local school system where the child is attending. • Calculation - Number of unexcused absences/number of school calendar days *Note: Number of unexcused absences is approximately 3% of 180 school calendar days (Standard Georgia school calendar year). * Note: For children in contractor’s care for only a portion of a school year, the calculation is pro-rated. * Note:Unexcused absences are defined by the local school system where the child is attending.
Scoring • 33% of Goal • 6 possible points • Pass/Fail
