  5. Our Programme Boards • Prescribing; • Mental Health; • Children’s; • Maternity; • Urgent Care; • Planned Care; • Long Term Conditions; • Primary Care Quality.

  6. Current state of health services in City and Hackney • Homerton University Hospital Foundation Trust (HUHFT) is our acute trust. They provide 70% of our hospital and community health services • East London Foundation Trust (ELFT) is our main mental health provider • We have 44 GP member practices • Local GPs are now running our GP Out of Hours Service as CHUHSE social enterprise.

  7. Our achievements • We are near the top of QoF for GP quality, and eye screening for patients with diabetes and for prevention of stroke. • Our “Tavi service” (mental health in primary care) were awarded Psychiatric Team of the Year. • Our Trusts are well regarded and financially stable
