  8. Section 1.About the CHIME FoundationHistory CHIME Foundation A hallmark of CHIME’s success has been a close collaborative relationship with the HIS vendor and consulting community. The CHIME Foundation was formed by the CHIME Board of Trustees in 1994. Incorporated as a separate nonprofit organization, the Foundation was structured to support CHIME’s programs and activities. In 1994, fifteen firms stepped forward to become the Foundation’s first group of member firms. Section 1. Contents

Section 1.About the CHIME FoundationFoundation Membership Criteria • Supports the purpose of the CHIME Foundation: To encourage, develop, fund and implement programs that foster the empowerment of CIOs and the advancement of the Healthcare IT industry. • Commits to working in partnership with CHIME CIOs to refine your company’s strategy and direction regarding the needs of the healthcare industry and can demonstrate this support with documented examples. • Demonstrates an established presence within the healthcare marketplace with a significant level of business activity related to consulting, recruiting, technology and/or applications within the healthcare IT industry. Section 2.Benefits and GuidelinesSpring CIO Forum The Spring CIO Forum is held in conjunction with the Annual HIMSS Conference and Exhibition and is scheduled for the Sunday before the conference starts. The Spring CIO Forum is a one day, members-only educational event and also includes a golf tournament and a welcoming reception the day before the program. Section 2. Contents

There are no registration fees for Foundation firms before the early bird deadline. Registration Forum Sponsorship • Foundation firms have the option to sponsor events at the Spring CIO Forum. This Forum features three days of CIO-focused education and networking events, including keynote speakers, track sessions, Foundation focus groups, social and recreational networking activities, and receptions. Registration Forum Sponsorship • Foundation firms are credited with sponsoring any events and/or items. • Fall CIO Forum sponsorships are available to all Foundation firms. Through the years, we have had great success with the Foundation firms’ participation in the program. To ensure the continuation of that success, it is essential to have Foundation firm representatives adhere to the Fall Forum Conduct Guidelines which are categorized as follows: Section 2. Contents

Foundation firms are encouraged to participate in the selection of CIOs for their online focus groups. Modest honorariums are paid to attending CHIME CIOs, whose organizations allow for such compensation, by the CHIME Foundation. CIOs may donate their honorarium to the organization of their choice. The Warehouse includes a variety of articles, including basic primers, research papers, policies, sample RFPs, sample job descriptions, and links to useful websites. The search-based organization of the IT Warehouse allows CHIME members and their staff to find relevant reference material quickly and easily. Results are returned ordered by relevance.   41. Section 3.CIO Membership CriteriaGeneral When you join the CHIME Foundation, you enter into a partnership with our CIO members. Therefore, we feel it is important that you familiarize yourself with our CHIME CIO membership criteria and benefits. Please note that these criteria and benefits are exclusive to our CIO members and are separate from CHIME Foundation firm criteria and benefits. Membership is targeted for those principally in charge of Information Systems/Services in the healthcare community. While those who qualify will generally be the CIO (the highest ranking executive responsible for information technology), those in charge of IS for major divisions and/or regions of large corporate or integrated delivery systems will also be considered for membership. The healthcare community will include delivery systems and payer/insurance organizations. Because of the rapidly changing healthcare landscape, these criteria serve as primary guidelines, which may evolve as industry and IS strategies change. Section 3. Contents

Section 3.CIO Membership CriteriaDetail Criteria • Associate Membership dues covers twelve months of Associate member benefits, beginning from the receipt of payment of dues in full • The firm who has been an Associate Member the longest will be required to upgrade to Premier level within 30 days of payment of dues in full • You Associate member dues can be prorated with the unused portion of the annual dues applied to the Premier membership dues • Should your firm decline to upgrade to Premier, your Associate Membership becomes non-renewable • Should your Associate Membership expire, your firm may re-apply for Associate Membership no earlier than 3 years • If re-applying in 3 years and there are no existing Associate Memberships, your firm may re-apply at the Premier member level Section 3.
