Market evaluation framework The two main objectives for this tasks were to: • estimate the potential demand of the HO services in Denmark, Italy and Spain; • analyse the main factors that could impact telemedicine development, in particular the role of public authorities. In order to carry out the HO market evaluation, PwC developed a specific framework.

Market evaluation framework: Tele - counseling The potential demand for the telecounselling service was quantified through the estimation of: • N. of reproductions (Hub); • N. of possible connections for each reproduction (Spoke). In a short-term hypothesis, the num. of HO possible reproductions was calculated through the mapping of the hub hospitals in Italy, Denmark and Spain, along with their potential networks. In a medium-long term hypothesis a new configuration of the potential market following the rationalisation of the providers will be considered (in particular with referral to the high speciality hub hospitals).

Estimation potential for HO demand in Italy The potential estimate for the HO Neurotelecounselling service in the Italian market was conducted by mapping all the structures in all the Italian Regions that can: Provide neurosurgery services Make use of the neurosurgery service The basic assumption of the mapping activity is the connection of a hub hospital with the spoke structures within the same province.

Estimation potential for HO demand in Italy The results of the estimation carried out by PwC are the following: 686 hospitals identified of which 569 with an ER, considered spoke and 88 hub hospitals. This data represents the potential market of the services offered by HO in Italy for the Teleneurocounselling area.

  11. Estimation potential for HO demand in Denmark The same methodology was employed in Denmark. The potential new users were identified. The table below summarizes the data related to the number of hub hospitals and spokes for every region in Denmark. For each hub several telemedicine workstations can be applied. In cardiology for the County of Funen, at least 3 telemedicine workstations can be used: 2 transportable ones at Odense University Hospital and 1 at the Svendborg Hospital. These could be used full time by a health professional at other (spoke) hospitals in the region during the week. In the Business Plan the cost and savings are calculated for the County of Funen.

  12. Estimation potential for HO demand in Spain The Spanish business case is based on tele-counselling for oncology, haematology, nephrology (dialysis) and radiology in the existing pilot sites. Market evaluation framework: telelaboratory Telelaboratory framework In order to estimate the potential market for the telelaboratory, we identified all possible laboratory networks and the number of POCT that can be used in Homecare Assistance (ADI) and in Homes for the Elderly (RSA). The number of POCT in elderly homes is going to be equivalent to the total num. of structures identified 1.928. RSA The minimum number of POCT to be used in ADI corresponds to the number of Socio Sanitary Districts, meaning 807. ADI

  16. Conclusions Computer and telecommunication industries have had a profound effect on the way medicine is practised and healthcare delivered. Although telemedicine is not a universal solution for all of our healthcare delivery problems, it can help minimise time and distance, which are extremely significant barriers to the delivery of healthcare, especially in non-metropolitan areas. As with all new initiatives involving change, both health professionals, patients and policy advisors will be less resistant to the concept of a telemedicine services if it can be demonstrated that technology can improve the quality of care and meet clinical needs and contain costs.
