OMC Services • Pregnancy Testing • OHP application assistance / Expedited OHP approval • Prenatal needs assessment & referral to PNC • Referral for maternity case management/home visiting services, WIC, Dental care, and other prenatal services as needed

OMC tracking forms Each site sends a one-page tracking form to the state OMC office for each client. • Problems: • Required faxing and maintaining reams of paper documents • All information had to be manually input from paper forms into a database • Required OMC staff to call/email local sites to get any missing information
