  3. Basic Information • 16 years old • 10th Grade • E.D. • Resource • Standard Diploma • I hope to graduate by June 2010

  4. History • McGaheysville, Wilbur Pence, Turner Ashby • Emotional Disturbance • How I react to people has at times been a struggle for me.

  5. Strengths • Football • Weightlifting • Swimming • Reading • Math • What I am proud of: My HVAC evaluation at MTC, My dedication to lifting in the weight room this year, and the friends I’ve made so far at TA.

  6. Parent/Student Comments and Concerns • Play football for TA, Graduate, go to college,and $Plumbing$. • My parents would like me to graduate and go to college,and make good money.

  7. How My Disability Affects Me in the Classroom • It affects me because when it is time to get work done, I’m trying to socialize with people. • When I don’t pay attention, or when I am talking to another person, teachers have to repeatedly keep reminding me to stay on task. • Listening to music while working, and extra time to complete tests and quizzes are my accommodations • When I change facial expressions, teachers could remind me to calm down. • Study a lot more often, turn in homework as soon as possible, and playing less video games will help me be successful.

  8. Social Emotional • I have pretty good relationships with teachers. Some friends I tend to get along with really well . Some other people I choose not to get along with. • My disability affects my emotions and the way I react to people. • Sometimes I get mad easily, and people know how to set me off. • How I cope with stress • Lift weights • Listen to music • Run • Squeeze a stress ball

  9. Transition • Become a plumber • Graduate College • Follow Directions (I can when I want to) • Work on houses every once in a while • Cashiered at the Rockingham County Fairgrounds • Cook, clean, laundry, and do chores

  10. Transition Service PlanInstructional and Related Services • By June 2008, I will pass all my classes with at least 72% as measured by report card or interim grades. • Community College or a 4-year university enrollment after high school.

  11. Transition Services PlanCommunity Services • By August 2008, I will complete requirements for 4 classes during boy scout summer camp with 100% mastery as measured by receiving 4 merit badges. • After graduation, I will continue to play basketball at church in the community.

  12. Transition ServicesDevelopment of Employment and Post-School Adult Living Objectives • HVAC vocational training at MTC next year • Full-time employment after graduation • I will have my learner’s permit and have completed the behind the wheel test by September 2008.

Goals and ObjectivesMeasurable Annual Goals • My goals are ignoring other students when insulted , staying on task during class, writing assignments down in planner, and improving measurement and tool identification. • All due by April 2009 • My progress will be measured by discipline records, resource checklist, teacher reports, and class grades.

  14. Accommodations • Test questions may be read aloud in a small group setting. • I may report to designated area in time of crisis or frustration. • I should sit in an area with least distraction. • Extra time may be given for assignments, tests or quizzes , not to exceed 6 week marking period. • Music can help to calm student or maintain focus at all teacher discretion.
