Purpose • To provide an introduction to CAF messages for demonstrator sites & suppliers • To explain the processes which CAF messages are designed to support • To show the content of CAF messages and how assessment tools can be mapped to them • To provide an overview of the technical approach and business analysis documentation included in the MiM • To identify areas where local requirements need to be defined • To discuss how to carry out this requirement work

Technical ApproachIG Access Controls • RBAC: • Role Based Access Controls are used for authorising access to system • functions and data appropriate to specific roles • LRS: • Legitimate Relationships are used for authorising user access to individual • patient records, where the user has a direct care relationship with the patient • Audit Trails: • Audit trails record users’ actions in relation to NHS CRS personal data • Sealed Envelopes: • Are used to allow patients to exercise choice about the level of visibility of • information stored about them

Technical ApproachConsent • Prior to accessing NHS CRS (Personal Demographics Service or Summary • Care Record), social care user needs to seek consent from individual: • Logged on session only • Until next assessment • Until end of care episode • Across any care episode • Filtering so social care users can only access specific documents – CAF • messages • Prior to sending copy of assessment/ care plan to Summary Care Record • user needs to seek individual’s consent to do so • If consent is not given, care professional may still send point to point

  41. Scenario Overview Background Ms Rachel Walters, a lady aged 46, has had mobility difficulties since a car accident several months ago. She visits her GP, Dr Alan Norman at Church Lane Surgery for a medication review, where she is also asked about how she is coping. Rachel tells him that she is having increasing difficulties with washing, bathing and other personal care. “How are you coping after the car accident?” “Actually, I’m having difficulty with washing, bathing and other personal care” Scenario Create Contact Assessment

Spine: ACS PDS SDS Create Contact Assessment Scenario After discussions with Ms Walters, Dr Norman believes that she requires social services help to assist her at home, and so completes a Contact Assessment. PDS is accessed for patient details. SDS is accessed for organisation and professional individual’s records. ACS is accessed to confirm permissions against the individual. Once the Contact Assessment is complete, this will be sent to Hebburn Social Services. Scenario Create Contact Assessment

Create Contact AssessmentCAF Information Set The model includes CAF Information Set components Rendered Example of the Contact Assessment form From within the HSCI Model, the Communications Analysis section provides a link to the object models. Your Contact Assessment, doesn’t have to physically look like this one does, but it will have the same CAF Headings from the CAF Information Set The Clinical Document, is a template to hold the details of a Contact Assessment Clicking on the Contact Assessment class model, provides access to the relevant CAF Information Set components. Utilisation of the CAF Information Set Where Information about the CAF Information Set can be found How this information is used to Render the forms The CAF Information Set are the recommendations on what should be collected as part of the Individual’s assessment The CAF Information Set can be carried within the message, provided that they are mapped to the headings Rendering examples are provided for suppliers but these are not mandated, so will need to be locally determined Scenario Create Contact Assessment

  47. Spine: ACS PDS SDS Receive Contact Assessment Scenario The contact assessment is sent from Dr Norman’s clinical system. Once the message is received into Hepburn's Social Services system, PDS is used to retrieve Ms Walters details, using the validated NHS number within the message. SDS is also used to check the details of which organisation/ individual the message has come from. ACS is also checked at this time also. Hebburn's System renders the message locally to ensure that Leigh Timpson can read the Contact Assessment details, including who the assessment is for, and where it has come from. Ms Walters explains that she receives some support from her son, but realises that in order to remain living independently in her home as she wishes, she will require some additional support services for personal care. Her decreased mobility also means she is unable to get out as much as she would like, and is at risk of social isolation. They complete the Overview Assessment together, with details entered into Hebburn’s Social Services system. A copy is made available to Ms Walters, and a copy is also sent to Dr Norman.
