Causes of Decubitus Ulcers: • Pressure • Friction • Lack of Cleanliness – sweat, urine, feces, wound discharge, soap left on skin
Causes of Decubitus Ulcers: Pressure results from: • Obesity • Casts / Splints • Bandages • Wrinkles in sheets • Staying in one position too long • Shearing force – skin rubbing on sheets
SYMPTOMS: Pressure causes: Decrease in circulation Increase in heat & moisture • Heat • Redness (Erythema) • Tenderness • Discomfort / Burning • Broken skin

Decubitus ulcers frequently occur over bony prominences: • Shoulders • Elbows • Hips • Sacrum • Heels • Ankles • Toes • Ears • Breasts

Prevention is the key!! • Assure clean, wrinkle-free linen • Encourage activity • Turn bed-bound patients every 2 hours • Massage around reddened areas • Provide back rubs • Keep skin clean and dry • Assess skin – report changes promptly • Limit time on bedpan

Special skin care needs of the elderly 1. Dryness, less elastic skin (decreased oil gland function/soap left on skin) 2. Skin less nourished - thin (decreased circulation) 3. Bed Bound – Immobility - Skin pressure - Rubbing on sheets - Sheets soiled / wrinkled
Treatment: • Relieve pressure on reddened area • Gentle massage around area – not directly on it • Special dressings: (Opsite / Transderm) • Lights to wound • Surgery if extensive ulcer
