Housing First - Social Services Model • The primary goal for each PSH tenant is housing stability • Social service provider does a thorough intake assessment before or immediately after a tenant moves into the building. • Based on the assessment, a support services plan is put in place for each tenant, with an overarching goal of housing retention • Once in residence, tenants are strongly encouraged to remain engaged with the case workers, but ongoing social services are not required of the tenants • Services provided on-site are complemented with access to additional services in the community provided by other agencies
The Summit at St. Martin’s - DC • 178 total units, including 50 PSH at 30% AMI • Social Services for the 50 PSH units are funded as part of the operating budget • Wait List for PSH units prioritizes formerly homeless individuals referred from transitional housing programs • Opened in 2010
John and Jill Ker Conway Residences - DC • 124 total units, including 60 PSH for homeless Veterans and 17 PSH for persons referred by the Dept. of Behavioral Health (DBH) • Social Services through contract with the VA and DBH • PSH referrals through the District’s Coordinated Entry system • Under construction – completion early 2015
HELPUSA at Walter Reed - DC • 75 total units, all PSH for individuals • Social Services to be provided under contract through the Dept. of Human Services • PSH referrals through the District’s Coordinated Entry system • Scheduled to open mid-late 2016
UNITY of Greater New Orleans • Most recent PSH project is Canal Street/Sacred Heart – 108 units, including 54 PSH. • PSH units are prioritized for formerly homeless Veterans • Opened in December 2014
