Helga E. Rippen, MD, PhD, MPH Deputy Senior Advisor National Health Information Infrastructure Department of Health and Human Services HL7 Meeting May 2004 National Health Information Infrastructure (NHII): Moving Toward Implementation
Overview • What is NHII? • HL7-EHR Functional Standards • HL7-EHR Interchange Standard • Future – Computable Guideline Exchange Standard
I. What is NHII • Standards • EHR for all providers and settings • Community health information exchanges
community Hospital Record Records Returned Laboratory Results Specialist Record Requests for Records Temporary Aggregate Patient History Patient Authorized Inquiry Index of where patients have records LHII system Patient data delivered to Physician Clinical Encounter
U.S. Hospital Record Records Returned Laboratory Results Specialist Record Requests for Records Temporary Aggregate Patient History Authorized Inquiry from LHII Index of where patients have records another LHII LHII system Patient data delivered to other LHII
HL7-EHR Functional Standards • Congratulations! • EHR interchange standard must accommodate the information needed to support the functions defined in the DSTU
HL7-EHR Interchange Standard • Goal: • Transmission of complete EHR between any system without information loss • Requirements • Information model must accommodate all possible types of information (from scanned documents to fully standardized and encoded records) • Arbitrary subsections of the record can be transmitted • Implementation should not require bilateral technical agreements (need meta-transactions)
Future – Guideline Interchange Standard • Goal: • Sharing fully computable decision support algorithms • Problem: • Every site of care can not muster the resources to develop a complete suite of decision support algorithms • Solutions: • Standard for encoding decision support would allow sharing and distribution of algorithms (i.e., printing press for guidelines)
