Medicare date says 2/3 of all Medicare patients have 2 conditions. • Examples of Chronic Conditions -: Alzheimer's disease and related dementia; Arthritis (osteoarthritis and rheumatoid); Asthma; Atrial fibrillation; Autism spectrum disorders; Cancer • Medicare payment for 99490-: There is a copay of $8 per patient per month to be collected, we do not charge you any upfront charges or any ongoing technology charges. http://www.chronicwatch.com
