Building the AP Repository • Assumptions: • Olmsted County will host the AP repository for SE Minnesota • Using a separate instance of the PH-Doc database • Account Administration tasks and issues here • Primary physician from one of the SE MN Beacon partner sites; • Mayo Clinic “M”/ Winona Health “W”/ Olmsted Medical Center “O” • Primary physician sees patient/student and develops AP • The AP is stored as a PDF document at the Beacon partner site (“M/W/O”) • Attending School is collected by M/W/O • Release of information process undecided • is obtained at M/W/O • Is obtained by school via paper

  3. Building the AP Repository • Data Flow: • M/W/O sends list of new APs to Olmsted GATEWAY • Nightly? Weekly? • Patient demographics (for searches and correlations) • Primary physician and/ or service area (updates via fax or future Direct) • Includes current school information (for creating school lists) • For each student on the list … • Gateway updates AP database (structured data elements) • Gateway requests AP Document from sending site (M/W/O) • Gateway stores AP Document in document cache (non-structured data) • Optionally M/W/O can use NwHIN Push • Includes same structured data as above • Includes PDF document

Using the School Portal to Access AP • Data Flow: • School nurse will sign into school portal • Using standard Web Browser and HTTPS • Rigorous Security and Authentication, Password Reset • School nurse will see list of students • Search by school and/or student name • School nurse will select student from list and … • View Action Plan • View MIIC Webpage (immunizations given and forecasted) • Complete INCIDENT REPORT and send to physician • What if student not found on list … • School nurse will need consent to search outside of Portal repository • NwHIN CONNECT Outbound Request to Beacon partners • If found, added to repository
