Goals of Program Reduce unnecessary admissions (and readmissions) Keep patients well-managed at home Teach/empower patients to take care of their heart failure Engage caretakers in management of patient’s heart failure Test efficacy of remote patient monitoring to keep chronic care patients well managed at home. Program is funded through August 2011

    4. Who are our patients? Target underserved populations: = 60 years old Medi-Cal Medi-Cal with HMO CMS/Self-pay Medicare (FFS) High utilizers of the ED Patients with history of readmissions within 30 days of discharge from hospital

    5. What does program include? Assessment in hospital by program coordinator (MWF at SGH, T/TH at SCVMC) 90 days of monitoring by Cardiocom™, with use of Cardiocom Telescale® Daily upload of weight and Healthcheck questions Weekly phone call from program coordinator and phone call follow up by Cardiocom™ Nurse Coordinator to patient and physician for variances in weight or symptoms.

    10. Hint •To create a flowchart, you can modify this example (copy & paste) or you can start from scratch to create your own flowchart. To add flowchart shapes In the Drawing group on the Home tab, click on the bottom “arrow” button next to the shapes, and find the Flowchart shapes. Click on the shape that you want. Use the cursor to create the shape where you want it on the slide, and size it as you like. To add connectors between the shapes 1. On the Drawing group, click on the bottom “arrow” button next to the shapes, and find the Lines shapes. 2. Point to where you want to lock the connector. 3. Click the first connection site you want, point to the other shape, and then click the second connection site. •To delete the sample flowchart and these instructions, press Control + A and then Delete.
