  14. Fee Waivers Students who receive free or reduced lunch are eligible to receive fee waivers • 2 Test waivers for ACT/SAT • 8 College Admission Application waivers • 4 SAT application waivers-Print from Collegeboard account • 4 ACT application waivers-Pickup from College Center • 1 NCAA waiver

Transcript Request Students are encouraged to review their transcript before it is sent off for college admission. • Complete transcript request form • Submit request form to Mrs. Oviedo in the Counselor’s Suite • Pick-up transcript from Mrs. Oviedo 48 hours after request has been submitted or have it mailed directly to the college.

PROM NOTES! • ONLY JUNIOR OR SENIOR STUDENTS MAY ATTEND • ANY STUDENT THAT DOES NOT ATTEND LHS MUST HAVE A GUEST FORM ON FILE AND BE APPROVED BY ADMINISTRATION • GUEST CAN NOT BE OLDER THAN 20 YEARS OF AGE • ALL DRESSES MUST BE APPROVED (see next slide) • NO TENNIS SHOES-JEANS- OR CLUB ATTIRE! • DRESS TO IMPRESS AND PLAN TO HAVE FUN!

SENIOR FEES • EACH SENIOR WILL SELECT THEIR OWN ACTIVITIES/EVENTS/PRODUCTS • A SENIOR CONTRACT MUST BE ON FILE TO PARTICIPATE IN ANY SENIOR ACTIVITY OR EVENT • THE NEXT PAYMENT OF $90 IS DUE OCTOBER 31ST. ALL PAYMENTS MUST BE COMPLETE BY DECEMBER 18 • ALL PAYMENTS SHOULD BE IN THE FORM OF CASH OR MONEY ORDER • NO REFUNDS!
