Chart Audits: Public Health • Audits are likely to be done to make sure information needed for state programs, grants etc is present to be reported on. Examples: Information contained in Client Details: DOB, Subscriber, Custom field value… Information contained in Services: Pay Source, Cost Center, Custom field values… Information contained in Charting: problem details completed KBS assessed specific actions (interventions)
Chart Audits: Public Health How do we make sure that information needed has been added especially if it’s not a required field? Reports!
Chart Audits: Public Health Self audits: build report to appear at the client level to make sure those fields are present • Clinician directed • Check after documentation completed • Build competency Agency level audits: • Run prior to main report • Run periodically through the year to follow up on data
Chart Audits: Public Health Audit Tool in Nightingale Notes to audit charts or movement in Nightingale Notes Client – Logs related to modifying and viewing records that relate to a client records AND to service records when no client is assigned. This includes: Client Detail Tabs Custom tabs Client Services including charting, vitals, supplies, and custom tabs, Services with no client OASIS, creation, edits, locking Payments in the payments screen 485s creation, edits, locking Verbal Orders Aide Assignments Supervision Visits Reports run from the Services or Client List
Chart Audits: Public Health Audit Tool in Nightingale Notes to audit charts or movement in Nightingale Notes Report – Logs related to running/viewing reports (Only includes reports run from the Client or Service context at this time) Data Exchange – Logs Related to sending or receiving client data (includes document requests, generating ccds, sending documents to Connect, and inbound P2P requests). Note: ** This will only return data for the agencies that are currently using the data exchange features. Those features have are not available in all states at this time. Page 4 of 7 Security –employee authentication (log in, log out, failed logins, re- authentication) and employee setup modification.
