  3. Process before 2010-2011 Season PAPER SCREENING TOOL FILLED OUT REVIEWED AND SIGNED BY THE DOCTOR FAXED TO PHARMACY ENTERED AND PUT ON HOLD RN CALLED PHARMACY FOR DOSE PHARMACY SENT DOSE TO FLOOR ADMINISTERED TO PATIENT PRIOR TO DISCHARGE

  4. Process before 2010-2011 Season PROBLEMS: DOCTORS MISSED SIGNING THE FORM SIGNED FORMS NOT FAXED TO PHARMACY ENTERED ORDERS ON HOLD MISSED BY RN DOSES REQUESTED AT TIME OF DISCHARGE PHARMACY SENT DOSE PAST DISCHARGE DOSE NOT ADMINISTERED OR DOCUMENTED

  5. New Process For 2010-2011 Season SCREEMING DONE BY NURSING AT ADMISSION APPROVED STANDING ORDERS BASED ON SCREENING (REQUESTED ANNUALLY) PHARMACY VACCINATION REPORT PHARMACY AND NURSING COMMUNICATION (TALK WITH PATIENT WHEN DESIRED) ORDER SCHEDULED AND SENT AS DISCUSSED PHARMACY FOLLOW-UP DAILY
