  2. •You will see the page on the right >> •Click on <<Register new camper>> •Select Lien Doan Gia Dinh from the Lien Doan List under Mien Dong Hoa Ky.

  3. Step 2 a: Individual Registration • Click “Register”, • Individuals will use your email address for your User ID - Fill out the registration form & click “Submit” To register other members in your family, click on “Copy”.

  4. Step 2b: Registering other members of your family (those who have the same address information) To register other members in your family, click on “Copy”. -The information will be duplicated. Please enter the name and other individual information as needed: -Name -T-shirt size -Scout level -Gender. -Etc…

  5. Step 2D: Medical form, Code of Conduct, Photo Waiver Form - Print & fill out BSA Medical form parts A,B,C (form is on registration website). -EVERYONE must fill out a BSA medical form: Boy scout, Girl scout, family member, friend of family, etc… We have to use BSA form since the camp site is of BSA . -If you are staying at camp, you must fill out the medical form. -Print & fill out Code of Conduct (on registration website). -Print & fill out Photo Waiver Form (on registration website).

  6. Individual Things to Turn In Every individual needs to turn in to your Lien Doan: –Register online under your Lien Doan name. –Payment to your local lien doan. Lien Doan will send lump sum check for campers to TT10. –Medical Form A, B, and C (Copy is due with your payment) –Signed Code of Conduct (Due with your payment) –Signed Photo Waiver Form (Due with your payment) Deadline to turn in the above with payment to Tr. Lam (check made payable to Lien Doan Gia Dinh): - payment of $220 if turn in by Feb 15, 2014. - payment of $250 if turn in between Feb 16, 2014 – April 15, 2014. - payment $280 if turn in between April 15 – June 01, 2014. (no guarantee to have all camp accessories that are available to other campers).
