Country examples of an evolved role of pharmacists The medication record and new roles for Pharmacists in France Dossier Pharmaceutique (DP) • An electronic record shared between pharmacists in France, developed by the national Council of pharmacists • It can be created for all beneficiaries of the health insurance once they have given their informed consent. The information regarding all medicines dispensed to the patient in the last 4 months in any community pharmacy is registered in the record, including both prescription and non-prescription medicines. • This tool helps the pharmacist to prevent iatrogenic effects caused by medicine interactions or redundant treatments. • A government order of 15 December 2008 has provided for the use of the DP in all French pharmacies. By early October 2009, nearly half of French pharmacies already operated the DP system. Towards a new role for pharmacists • A law passed in 2009 will allow further developments.. Within the framework of such collaboration, it will be possible to transfer some responsibilities, particularly from doctors to pharmacists. At the doctor’s request or with his consent, a pharmacist designated by the patient as his “coordinating pharmacist”, will periodically renew chronic treatments, adjust doses if needed and carry out medication reviews intended to optimise the treatment.

Country examples of an evolved role of pharmacists Pharmacy services in the UK • Pharmacies provide a series of services designed to target public health including stop smoking and sexual health clinics, weight management, diabetes, and cholesterol and hypertension control. • In 2009, the UK Government proposed to commence a series of vascular health checks for everyone in England between 40 and 74 years of age. Pharmacy is at the forefront of this service, offering a national screening process that measures weight, blood pressure and cholesterol levels. Those members of the public identified as most at risk are referred for further treatment to the practitioner most suited to provide treatment. • Pharmacy services are delivered under a framework « pharmacy contract » where pharmacies are paid depending on the range of services they provided.

Country examples of an evolved role of pharmacists Pharmacy Services in Italy The new pharmacy services (D.Lgs 153/09) • New legislation setting a list of obligations for pharmacies in terms of services to be provided (May 2009) – expected to be implemented towards second half of 2010 Integrated homecare • Delivery of medicines and medical devices • Preparation and delivery of artificial nutrition blends and painkillers • Delivery of hospital medicines through agreements with the pharmacy (so called “distribution on behalf”) • Delivery of nurses, physiotherapists and other health professionals • Consultancy on health issues (nutrition, usage of non-pharmacy products, healthy life styles) Health promotion & prevention • Involvement in national and regional health promotion campaigns • Possible even greater involvement in future vaccination campaigns (nurses) • Smoking cessation • Nutritional education (primary care analysis) • Periodical checks for patients’ groups at risk • Guidance on proper usage of medicines and medical devices Services under doctors’ prescription (and with nurses aid) • Prevention of cardiovascular deseases: hypertension, cholesterol and triglycerides, filing of internazional cardiovascular risk test • Diabetes: hypertension, glycaemia and filing of Tuomilehto test • Ulcer and gastritis: helycobacter pylori with Test HpSA • Test PT for patients under oral blood-thinning medicationtherapy • Telemedicine/elettrocardiogramm: ecg in the pharmacy and medical report from a remote cardiologist sent to the pharmacy via email • Telemedicine/holter monitor: device rental in the pharmacy and medical report from a remote cardiologist sent to the pharmacy via email • Semiauthomatic defibrillator for emergency!?
