  1. Orders for Withdrawal of Life Support in the ICU

You MUST complete both: • Doctors Orders to Withhold or Withdraw Life-Sustaining Treatment Including DNR, which MUST be signed by the attending physician within 24 hours • Progress Note for Withholding or Withdrawing Life-Sustaining Treatment Including DNR, which must be cosigned by the attending physician within 24 hours

  3. SELECT ANALGESIA BOLUS & DRIP – choose from: • Pre-configured starting dose, or • Provider-entered starting dose

  4. SELECT SEDATION BOLUS & DRIP – choose from: • Pre-configured starting dose, or • Provider-entered starting dose

  5. Enter Ventilator Orders(Consult to Respiratory Therapy)

  6. Resultant Orders (pre-configured dosing)

  7. SELECT ANALGESIA BOLUS & DRIP – choose from: • Pre-configured starting dose, or • Provider-entered starting dose: • Enter a starting rate in mg/hr

  8. Click the Link Click Open file

  9. Enter starting infusion dosage in mg/hr and press <ENTER> • Click COPY and close spreadsheet calculator

  10. Click OK Enter <CTRL><V> (this is the keyboard equivalent of pasting with the mouse) Titration schedule copied from spreadsheet calculator appears in lower box. Click Accept Order

  11. SELECT SEDATION BOLUS & DRIP – choose from: • Pre-configured starting dose, or • Provider-entered starting dose: • Enter a starting rate in mg/hr

  12. Click the Link Click Open file

  13. Enter starting infusion dosage in mg/hr and press <ENTER> • Click COPY and close spreadsheet calculator

  14. Click OK Enter <CTRL><V> (this is the keyboard equivalent of pasting with the mouse) Titration schedule copied from spreadsheet calculator appears in lower box. Click Accept Order

  15. Resultant Orders (provider-specified dosing)
