  2. Survivorship Pathway • Treatment summary completed • 1 month post-surgery • Copy given to patient & GP • Appointment booked for eHNA & LWD • Holistic needs assessment • End of active treatment • Care plan produced • Living well day • Held every 2 months • All patients invited to attend

  3. Living Well Self Management Course • 6 week programme • Facilitators:- Clinical Psychologist Breast CNS Patient tutor • Format:- Breast patients only, 8 – 14 participants Six 21/2hr sessions (1 session per week) 3 months follow up 4 courses per year • Referrals:- Self referred from Living Well day Breast CNS following eHNA Psychologist

  4. Progress to date • 9 courses completed • 10th commenced 7/7/2015 • 83 attendees • Pre & post QLACS • Quality of Life in Adult Cancer survivors • Recommended by NCSI

  5. QLACS • Reduction following intervention Negative feelings Physical pain Cognitive problems Fatigue Family related distress Appearance concerns Distress over recurrence • Unchanged Social avoidance Financial problems • Increase following intervention Benefits of cancer
