  13. Nerve and Tendon Transfers –Historical Evolution and Modern Concepts Andreas Gohritz Invited Presentation Schwerin, Germany April 2013

  14. History of Peripheral Nerve Surgery in EuropeInstructional CourseFESSH 2013 • Chapter with Lee Dellon • Chairman: L. Dahlin, Sweden

  15. Adolf Stoffel (1880-1937) – Pioneer of Peripheral Nerve Fascicular Anatomy, Selective Neurotomy and Nerve Transfers Andreas Gohritz, Lee Dellon* Basel, Switzerland, Baltimore, USA*

  16. Adolf Stoffel – Scientific Work Anatomist / Surgeon, Heidelberg University Nerve cross-sectional studies since 1909: • “Nerves are unlike tendons.“ – consist of Nervenbahn = nerve railways • Specific motor / sensory fiber distribution = Topographie → “Stoffel operation“ (1911) (= selective neurotomy in spastic extremities )

  17. Wrist Extension Reconstruction by Brachioradialis-to-ECRB Wrist extension-driven kex grip in 66-y-old patient (IC group 1) 47 years after spinal cord injury

  18. Orthopaedic Operations • Co-authored by Stoffel with his mentor Oscar Vulpius (1867-1936 ) • 3 editions (1913, 1920, 1924) • Numerous innovative orthopaedic techniques, especially for paralyzed patients

  19. Radial-to-Axillary Fascicular Nerve Transfer (before 1910)

  20. Stoffel`s Approach • Intraoperative electrical nerve stimulation → Safe identification of dispensible donor fascicles at level of recipient nerve damage or below

  21. Needle Electrode / Testing Apparatus

  22. Conclusions Stoffel´s example shows • Surgical progress always needs anatomy study. • Development frequently by persistent work against mainstream thinking. • Careful revisiting our ancestors`work may reveal old ideas that were never disproved, but only forgotten and my be worthy of being “re-thought” in new context / audience today.
