Certified Division Order Analyst “CDOA” All Division Order Analysts with a degree and 3 years of experience, or with 5 years experience, are eligible to take the Certification Exam to become Certified Division Order Analysts ("CDOA"). 19. NADOA Model Form Division Order Change: The 28% rate for Federal backup withholding was recently extended until December 31, 2012. To avoid the need to continually monitor for revisions to the NADOA Model Form Division Order, the NADOA Board has voted to alter the language on the bottom of the Model Form. Please review your company’s adaptation of the Model Form for inclusion of altered language. The newly revised Model Form is available on the NADOA website (www.nadoa.org). Federal Backup Withholding information may be found at http://www.irs.gov/pub/irs-pdf/p1281.pdf Rate/Percentage is 28%, effective for all subject payments after December 31, 2002 until December 31, 2012.
