  5. Dementia and Elderly Care Conference • What are 3 achievements in the hospitals? • Dementia Friendly Wards • 90% of over 75s admitted are screened for memory loss (target was 75%) • Length of stay has reduced and readmission rates have dropped in those referred to Liaison Psychiatry Service
