Restoring lost tooth structure • Most common Options: • Gold • Amalgam • Composite
Advantages of Amalgam • Ease of placement-tolerates slight moisture • Good durability • No shrinkage (expansion) • Easy to establish proximal contacts • Low post operative sensitivity
Disadvantages of Amalgam • Poor color match • Expansion • No bond to tooth structure • No early strength
Advantages of composite • Early strength • Beautiful color match • Bonds to tooth structure
Disadvantages to composite • Very moisture sensitive • Post-operative sensitivity • Leakage in the proximal box • Difficult to establish proximal contact • More steps
Steps we follow in placing a composite restoration • Moisture control-Rubber dam, parotid blockers, assistants • Complete decay removal-Caries detection dies • Disinfect the preparation-Glumma, hurriseal • Prepare the surface-Self etching primers • Contact closure-bi-tine rings and contoured matrix bands • Build the box and floor first and cure • Build out the remaining, cure in layers • Shaping -Raptor bur sets. • Checking the occlusion • Polishing-gold knife
Self etching primer to preparation, wait 20 seconds then dry vigorously
Fill the remainder of the preparation, cure 20 seconds then remove bands with hemostat
