Prior Authorization RequestCase Notes & Attachments • When requesting a new Prior Authorization, a case note OR attachment is REQUIRED. • eQHealth reviewers will utilize case notes to communicate with providers through a PA request case. • The ‘Action Required’ tab located in the provider portal is where providers can find cases that require additional attachments or information.
